Of course we won't know for sure until the day of and there's always room for a plot twist in the land of Mofftiss, but I really think you're on the right track. If Mycroft is there, he'll use words to restrain John rather than force. He's just that kind of guy.

Indeed. He'd be standing there terrified and desperate to help, but hating himself for his injury and his own helplessness. "BAFTA" doesn't even begin to cover it.

Good points about the drugs and misidentified bodies. We already know they like to drop hints. (This is partially Moffat, after all.) We had the Moriarty hints dropped in series 1 and I wouldn't be surprised if there have been subtle hints in this series too.
I like the idea of Sherlock being in a coma and possibly brain damaged. Definitely a twist.
I also like your theory for a Mrs. Watson being slipped in between series 2 and 3. I can't picture this Sherlock going to John's wedding like in Richie!Holmes.

It often makes me wish for plots that never happen, but I suppose that's what fanfic is for.
And while we're at it, here's another: I wonder what would happen if, in this version, Mycroft wasn't actually in on Sherlock's plans? Can you imagine his reaction to seeing his little baby brother alive again for the first time? WANT.
