i haven't had top surgery yet, but i have wanted them gone for as long as i can remember too.
i think it is the fear of the unknown that may be causing you to feel this way.
here are a few questions to consider when going through top surgery.
do you feel good having breasts? I mean if you have to find reasons to love them, it seems as if they are not right for you.
would you be ok with having them for the rest of your life?
do they look "right" to you when you look in the mirror?
are they part of your sense of "self"? 10 years doesn't sound like a phase to me, if they have caused you physical/emotional distress then it is definately not a phase.
