I just looked at the book. It's not very helpful at all. She just says use whole milk, introduce it gradually, and watch for allergies.

I don't remember, is your V on soy or cow milk based formula?
if cow then you could probably start with one cup of milk instead of formula and then later substitute more. (sorry, don't have the timeline and that's probably what you're looking for)
if she's on soy then you probably would want to use soy milk.
one thing you might try is goats milk. it's supposed to be more easily digested than cow's milk. even if she's been on cows milk formula the proteins there are already partially processed so goats milk may still be the better choice.
ooh, also look for organic, non-growth hormoned milk. They ~say~ it doesn't cause problems but I don't trust ~them~. :)

E was on formula from his second week. We brought him to the doc before giving him any dairy....Doc gave him an allergy test, and it turned out that he had a major reaction to dairy.

My advice? Wait for the doc.

If she has been doing ok on other dairy foods, IE, yogurt and cheese, I would just start a couple oz whole milk in a cup, then if no problem give her a little more milk each day and less formula.
Possible problems include vomiting, diarrehea, constipation, rash.

Unless she has had trouble with milk based formulas I wouldn't anticipate much problem.
