Sam was by this time awake and he seemed to be staying there for now. Bobby decided to cede to Dean’s wishes and get the younger brother inside first. He wasn’t sure it was a good idea to un-strap Sam, but the young man was heavy and there was no way that Bobby could actually fit the stretcher through the stairs and in the door

Though Bobby knew that he shouldn’t leave the boys, he had a mess to clean up at home. Once he was sure that Sam was not going to check out on him completely, he started driving back to South Dakota. Once there he quickly torched what remained of the building (Hey, there would be time to do a through job later, Right now the boys needed him), and grabbed the impala. Dean would hate it if his baby was left alone for too long, and the ambulance was hardly an inconspicuous ride

OP here: What a wonderful little story! You picked up on so many of the details I didn't mention but did think about - and even some of the ones I didn't. (But now make perfect sense.) The stretcher, the crutches, the Impala being at the cabin, Rufus' booby traps - love the bit about the fireman's carry. I also love how Bobby kept his mind off his own problems by caring for the boys.
I do think it would be the most natural thing in the world for Dean to store Cas' coat in the Impala. And I think they all miss him terribly, even if they have been too busy to show it. (That's show for you. That's why there are these fix-it memes.)
