That's what you'd think, but none of that is info I ever gave them. My best guess is that all this can be looked up via your SSN.
What was so scary about those security questions is that they were generated in a matter of seconds after I entered my information, so companies apparently can have instant access to all your history and records. Yikes.
Edit: Also, the address I gave Square was my new apartment address. I didn't give them my (parents') Cornwell Drive address that came up on the question, but they knew it anyway.

Last month I applied for a Square card reader for my business. In addition to my SSN (a given) it also asked me to answer the followign questions:

It new all that from my name, birthday and social. I was creeped out, to say the least.
