My approach is to screen all anonymous comments, along with recording the IP of every commenter. I've caught "anonymous" folks a time or two with that technique, especially if I notice that they're using the same IP address for both their anonymous and non-anonymous comments.
On top of that, though, I also make sure that nothing I post publicly is something that I wouldn't want anyone in the world to be able to read. I think of it much like publishing. I use multiple "friends" filters to manage who gets to read what, as well.
The other useful thing about screening anonymous comments is that some such commenters will be less likely to comment if they sense that there's some sort of barrier.
You know, I am getting all too addicted to LJ. If I started going private, maybe it would necessarily cut down my time spent in front of this screen...
