button holes are fairly standard sized -- most shirts will be able to give you a reference. you're right that a dime is smallish and a quarter is largish -- either would be ok, it depends on what look you're going for. nickle is nice an average/conservative.
the problem with the chain ones is it's almost impossible to put them on yourself (or at least i've found it so). the commercial hinges are fine -- i prefer the solid torpedo ones, because the other ones just feel too flimsy to me. soldering them on is a trick, because you have to make sure you don't heat them too much, or the mechanism either loses its springy, or seizes up altogether.

I prefer flat to convex, but I've seen concave.
Dime is too small. Quarter I've seen, but it's kinda gaudy. A little bigger than a nickel is the right size, IMO
They're pretty standard in size, I'd guess. Certainly the torpedo hinges sold commercially will fit them!

I like the hinged bar type.
Generally they are either flat or convex, because I think concave would sometimes catch on stuff.
I think about the size of a nickel (maybe a little smaller) is best.
