RE: people not being good Samaritans - it's pretty much human nature. No one wants to be responsible if something happens while they're trying to help. They may get sued or something.
I was once at a restaurant with Ian, who at 6'4" is a full foot taller than me, and I had to perform the Heimlich. Funnily enough, all the patrons looked away uncomfortably and all staff fled the dining area. Fortunately, I've taken CPR and First Aid classes... anyway, it's pretty hard to get leverage on a person who's a foot taller and at least 30 pounds heavier than you.

Honestly, in a situation where you have to jump one way or the other, that's never crossed my mind. Just not in my DNA, I guess.
Your story about saving Ian reminds me of a story a woman from London related to CNN. She was on a bus full of passengers when there was an altercation and a man got shot. In the confusion, the gunman got away. A bus full of people and nobody tried to stop him. As the man lay there bleeding, people were behaving in a way that she described as similar to how you characterized the people in that restaurant.
Pretty amazing. It's beyond passivity at that point.

He fell off the 3rd story balcony at his apartment during a party.
No one could figure out how he fell, but he had cracked ribs and such. Thirty minutes later when he "kinda" recovered, we asked him how he fell. He said, "I leaped, I was trying to catch my cigarette, duh."
Like we were the dumb ones. Who jumps off a balcony to catch their cigarette? This is so much MORE than basic physics people!
