Below, I've highlighted those that seem relevant to your question or might have the people you're looking for. There may not be a community specifically for it, but there will be individuals who will respond.
ftm is the big FTM community with moderated membership. It can be awfully combative in there, unfortunately, but that's where you'll get the most responses. The same can be said for transgender.
_ftm_ the the open FTM community, with fewer members but also a lot less drama.
glbtq_disabled seems an excellent place to ask. In the userinfo for that community they explain that they take a very broad view of what "disabled" means. It is meant to be all-encompassing.
There is basically one URL to use to check up on one's LJ, and it's (myusername).livejournal.com/friends. It's called your "Friends page," and it contains the latest entries in communities and individual's journals you've chosen to "watch."
When you're reading some interesting journal or community on LJ, you can click Watch or Add as Friend at the top of the page, and then you'll be given a choice to optionally add it to feed into your friends page. I always add it, or else I'll never see the content in my daily LJ check-ins.
