Hardcore mousing help needed

I've been having major problems since december with my hands generally resulting in carpel tunnel like symptoms. This week I went off the nerve pain medication I was taking because it was making me way too sleepy. What this means is that I really need a good ergonomic mouse for work and I'm looking for recommendations

I use a Logitech TrackMan Wheel. It's a track ball instead of a mouse, so you control it with just your thumb. But it also has a scroll wheel.

The ball takes some time to get used to, but in the end I like it a lot. A friend got one because of *his* carpal tunnel, says it helps him.
