Jim, I feel your pain. I was right at 400 pounds and finally said 'I can't do this any more' and decided on gastric bypass surgery. I'd dieted for years and between the diabetes (insulin for ~20 years) and heart issues, I'd managed to gain weight and end up part-time in a wheelchair.
I'm down 80 pounds or so now and still dropping - hopefully ending up below 250 when all is said and done. I know this isn't for everyone, and if it weren't for Medicare, I'd never have managed this.
Biggest thing that made a difference pre-surgery was cutting out all carbs and most fats. Not fun, but wow...
