I think the difference arises from the fact that insurance companies will only pay a percentage of a claim put in by a doctors office, so you are paying the actual amount, and they are over charging the insurance company so they get reimbursed the same amount.
Also, one of my doctors bills me for the difference between the insurance reimbursement and his rate, and the other one appears not to (since I haven't gotten a bill yet). I'm not sure now the deductible factors in either. Maybe they'll reimburse at a higher rate once I've used up my deductible?
