I thought the homicidal puppets on Smile Time (from an episode of Angel, season 5) were pretty good, though they didn't rip anyone's throat out.

My favorite would be the rabbit in HOLY GRAIL, of course, but a close runner-up would be the Skexies from THE DARK CRYSTAL.

Still, I must confess my first thought was the shudder-worthy flashback from the Move That Must Not Be Named. It's one of Peter Jackson's and there are many things done by puppets that should never be done by puppets. Ever.

Omg, I went to imdb to find that Peter Jackson movie and ... O_o Why? Why do that to puppets, Mr. Jackson?? I think watching it would scar me for life.
