I got quite a few migraines that fit your description exactly in my early 20's.
One doc told me it was possible due to unregulated or poorly regulated blood pressure in my brain. They gave me cafregot I believe.
If I took the cafregot and laid down in a dark room, the headache got much better. Sometimes I had to wait an hour and take another dose.
Can't say I ever had great visions, but if you are good at transmuting pain into other stuff (which you are), I could see making that trick work.
I seemed to grow out of the headaches. Only had a couple or three like that in the last 15 years and they don't last. Mine were definitely stress related.
