When I read the initial report, I had to read it twice. I simply couldn't wrap my head around the incompetence that would have to be in play for this to have happened.

I'm going to offer up a third alternative; a military that is so stretched, so exhausted, so put-upon that a mistake of this magnitude isn't merely possible, it's inevitable.

No, that doesn't make me worry any less.

Avindair, your third alternative strikes me as the most likely mechanism in play here, and I'm embarrassed that I didn't specifically call that one out.
