At first I thought the character might be Naruto. I only realised it was Gaara when the sand was mentioned. Anyway, I'm not sure angsty is the word I'd use to describe this. More like a very crazy trip through the workings of an insane mind. The bits in italics and bolds work wonderfully, popping up even when you don't expect or want them there and making Gaara's schizophrenia all the more real.

Yes, crazy-Gaara for the win! Someone gets the italics/bolds! I thought it might be weird, 'cause I showed it to my dad and he was like '...wtf?' I feel so accomplished now :D
