Was RTD always this bad, and the excitement of the show's return disguise it...or did he get worse over time?

I don't think his episodes with Nine were that bad, but once Ten showed up and they started pushing the Doctor/Rose angle...he got that bad. It's essentially what changed between Geoff John's time writing Stars and STRIPE and JSA. It went from, "It's really neat I get to work in this usiverse," to, "Now that I'm here, I should leave my mark...EVERY-FRICKIN-WHERE!" The show stopped being about Doctor Who after Nine and became about RTD's Love of Doctor Who.
