Once from internal heat. caused by a internal transformer that was getting to hot while the device was in "stand-by" and plugged in.
Two other times, it was actually caused by the sun. one device was on the dash of a parked car and the heat warped the crap out of it. the other was in the path of the sun beam through a window. and over time it must have gotten to hot. that device was also black plastic.
Well it wasn't plugged in and wasn't sitting in the sun. It was sitting on my husbands desk with the cord wrapped around it and was in the corner of the room. There are two windows in the room but both of them have heavy curtain that block out the sun.
