Ulquiorra was born into a rather distant branch of the old-rule
Monacello family. His Moncello family mother was married off to a
member of a related Spanish mafia in Barcelona as part of a business
deal. He lived an uneventful young childhood, pampered by his parents
and living every pleasure of being the only child of his well-off
mother and father. He was instilled with a strong sense of order and
social structures and politeness in preparation for becoming a high
ranking mafia member. His father’s brother was the don of the smaller
mafia group, so Ulquiorra was raised to act correctly. But this
seemingly perfect world came crashing down when both of his parents
were killed in an internal dispute over the heir to their family.

The new rule of Ulquiorra's family sent him to Reggio Calabria to get
rid of him. He was given to the Monacello family whose blood he
shared. But instead of enjoying the same lifestyle he had in Spain, he
was used as a test subject in the Monacello family's genetic
enhancement experiments. The management thought the otherwise useless,
weak ten year old boy would die from the stress. But Ulquiorra was
stronge than that, shutting away the weak and useless spoiled little
boy that he was. He dawned a cold, emotionless exterior to protect
himself from the pain he endured as an experiment. The doctors branded
him with the experiment number four, which was marked with a tattoo to
his chest. The scientists joked that it was an unlucky number, and
also gave the cold boy tattooed lines running down his cheeks like
tears mourning his misfortunate fall.

As an experiment, Ulquiorra was given new abilities the doctors
thought would aid in making him the perfect ruthless killer for the
Monacello name. The results were keen eyesight, speed, and skin that
was unusually hard to cut. The boy proved to have much potential as a
weapon, and was trained to be the perfect killer from the time he
recovered from the changes to his body. Through years of practice in
the lab, Ulquiorra ended up being a very precise shot because of his
eyes, and a skilled fighter due to his speed.

Before he knew it, Ulquiorra was caught up in the growing struggle
between the Moncello and Vescovo families once he was let out of the
labs and onto the streets at the age of fifteen. Owing a sort of
rebirth and new purpose to the family, Ulquiorra continued to feel
obligated to put all his loyalty and energy into serving the Monacello
family. He was already twisted into the family business on the streets
when the new don took power, and followed new don without question,
perhaps hoping that the don will remember him and one day reward his
loyalty with a position in which he can better serve the family and
regain the power he once enjoyed in his own family long ago.

Ulquiorra steadily worked his way up the ranks of the Monacello by
being efficient, cruel, and loyal. These qualities were noticed by
another man working his way up the ranks of the Monacello family who
reached the rank of caporegime. This man took Ulquiorra under his wing
for his skill and his potential to be dangerous. Ulquiorra at first
did not trust the man, who was not even related to the Monacello
distantly. He harbored a mild dislike for the ruthless man, but
followed his orders perfectly nonetheless. But that all changed rather
quickly when Ulquiorra recieved a hit that he could not complete.

Repeatedly beaten and broken by his target, whom he foolishly started
to have feelings for at some far corner of his cold heart, Ulquiorra
suffered extreme shame. He tried one last time to kill his target, but
that too only served to break him further and end what was already a
devastatingly abusive relationship.

Ulquiorra briefly returned to Spain, desparately wanting to be
stronger than he was. After a month of training, he returned to Reggio
Calabria to work towards regaining his family's trust and his own lost
honor. This led to him being involved in a large mass bombing strike
against Vescovo and the AMC. He was arrested and found guilty, sent to
a special ward since he as deemed extremely dangerous by the then AMC.
He spent the last years in prison, and recently was released as
conditions deteriorated in Reggio Calabria.
