Would anyone be willing to create an image for me?

I am taking New Testament Survey [Yes me of all people - I know it’s a scary thought]. Well at the end of the semester we have to hand in our folders. Since I am typing all of my notes, rather than writing them I have to have a cover sheet. I know, I know I can just make to simple blank one that has
