John is dosed with one of those 'go mad and die' concoctions popular in fiction, resulting in massive hallucinatory flashbacks and him running wild around London, dying slowly and unaware. Lestrade and Sherlock have to figure out he's been dosed, track him down and keep him from doing anything he won't be able to live with, find an antidote, and do all this before John dies, alone, hidden away in an abandoned building and determined to protect himself violently from anything that gets near him.
