Sherlock starts complaining about a vague, intermittant but chronic pain--back pain, maybe, or general joint pain. Sometimes he can ignore it and sometimes it's bad enough that he can hardly get out of bed. Mycroft, and even a few of the Yarders, suspect he's shamming as a way of getting access to drugs, but John and people closest to Sherlock recognize that the pain's genuine--but they can't figure out the underlying cause. And meanwhile poor Sherlock is trying to muscle along without anything stronger than paracetomol, resentful of his "transport" breaking down like this and ashamed when he has to ask John for help with some daily tasks. And perhaps daydreaming about how just a little sniff of cocaine might help the pain...

Could end with figuring out the cause in some weird House-like way (oh god Sherlock whacking people with a cane) or not; I'm mostly interested in Sherlock being physically dependent on others and/or facing down the serious prospect of a relapse.

TL;DR: Sherlock deals with unexplained chronic pain.
