He wasn't wrapped up in a blanket or strapped in or IVed or anything, just kind of artfully flung across the stretcher and being towed away. It looked so damn fake to me.
If he were dead, they'd have covered him entirely with that blanket he has tucked under his head. If he weren't dead and he were that injured, he'd be strapped in with a backboard, IVs, blanket for warmth, and they'd have certainly put his arm on the damn thing. Either way they would have strapped him to the gurney before towing him off, because having a patient fall off the stretcher is, you know, bad.
It could be that rule-of-cool is going on, but I hope not. I hope it's because Sherlock was faking it for observers and wanted to make sure observers watching would recognise him and think he was badly hurt.
It would be so much cooler if it's Sherlock being clever rather than the writers being stupid.

IMHO and this is *pure* speculation just based on seeing the pic and my own thoughts... Likely it's Mycroft's doing in part and also just being "cool" and "OMG ARTFUL INJURY!" rather than anything else.

Well, he could be faking it like your hopes, but I know that when I was taken away in an ambulance, they did not hook up the IV until I got inside the ambulance.

This review is fantastic, it manages to almost totally avoid spoilers while being descriptive enough to make me physically ache for the moment Sherlock finally returns! Thanks for sharing.
