As for the title, that's a little hard. Since this is based on Tsuna's death and you only show 2 pages. . . . . errrr how about Facade? The word basically means, a deceptive front put on by a person to fool someone. Sounds more like Mukuro, but that's all I can come up with since I've only seen 2 pages.
As for the cover, you should base it on the main premise of the doujin. Since this is about the death of Tsuna then something like a coffin, or something that represents Tsuna would work. A image with Hibari and Tsuna would also work. If you decide to stick with the title Facade then a picture of Tsuna holding a smiling mask will match the title, although there's really no mask in the doujin. . . . . is there? XD

Ahem. Before I get too excited blabbering in my comment... How about a bittersweet cover of Hibari, in a subtly moe pained expression? And pastel coloured background (of the sky + cloud + grass?) for added effect (melancholy). Soft, pastel colours would suit the poignant mood/theme of your story, I hope?
