During the Hiatus, Mary and Watson do conceive a child. However, instead of Mary dying in childbirth, it is Watson who dies while Holmes is away. Mary is left widowed, with their small child to raise.
Holmes discovers this upon his return to London and, though devastated by Watson's death, obviously isn't just going to let go of the last piece of Watson he has left. And Mary, for reasons up to the writer, has little in the way of substantial family support -- her parents have died, etc.
Can Holmes and Mary make a marriage of convenience work?

It's a difficult beginning. And hell, if my husband's best friend had the gall to be all "LAWLZ! I JUST FAKED MY DEATH! HOW ARE YOU?" after he died, a slap would be the least of his worries.
