So I have a default eye replacement, but my sims' eyes seem to have converted back to the maxis eyes, and I have no idea why. I mean, possible I inadvertently downloaded another default eye set, thinking it was custom, but would that result in the maxis eyes showing up

The disappearing object thing sounds like a hack conflict. Remove your hacks and delete your groups.cache file and load the game to see if it that fixes it.
Not sure about the maxis eye thing though.
