You could learn how to make your own shoes. Or, as I hear, there are shops that can get you shoes in a custom range of sizes and I have no idea what they are. I suppose amputation is not that far off from it though.

My mother is a size 5. Her parents attempted to bind her feet when she was a kid and it was only partially successful. If it had been totally successful she'd probably be a size 3 or 4. She has managed to get shoes in the past, but might have trouble now. I'm a size 6 in men's sizes, which is a bit difficult to find but often possible. The last time I bought shoes was in the UK where there is no difference in the numbers of men's/women's/children's sizes, but the shoes in the store were just lined up in order of size, and as the numbers got smaller the shoe population became more dominantly (but not completely) feminine and then kid-oriented.
