when someone says, 'i wear my heart on my sleeve' what does that mean?

That you are sensitive? Everyone knows your feelings? You get hurt easily? help me out here.

Yeah it kinda means that, like you are honest about your feelings and everyone knows what you are feeling because they are so emotional/expressive of said emotions (hence heart on sleeve like you can see the persons heart/feelings). I hope I explained that well I know what it means but sometimes its hard to describe things to others.
