I should add--I almost never buy used clothing solely because it's damn hard to find anything worth wearing in my size (not that I'm huge or anything...I just think fat girls keep their clothes!). =)
New shirts or pants: I might wear before washing. I used to say, definitely wear it before washing, but I don't like the idea of all the chemicals they put on everything.
New underwear: definitely wash first.
Used: I don't buy much used because I get frustrated since I'm an odd shape, but if I was given something used or bought something used, I would wash it. I am allergic to most laundry detergents, so I can't risk getting that rash again!
I heard David Sedaris on the radio-- he got crabs from wearing a pair of thrift store pants he neglected to wash first. And a friend of mine got bed bugs from a thrift store. I would have never thought of that before, but now I can't get it out of my mind.
My old friend got a nasty case of scabies wearing thrift clothes without washing which still makes me twitchy about even trying on thrift clothes, but I suck it up because thrift clothes rock.
I did my best to answer within the constraints of the questions, but it's really more complex than that.
1. Wait six months to wear it, letting it break in in the closet first. I have no idea why I do this. 2. Wear it without washing it -- I don't believe in germs.
If I'm buying something new for a specific really super special occasion, then I do NOT wash first for fear of it getting ruined in the wash. Then if it gets ruined at least I got to enjoy it for one special day. Otherwise everything gets washed.

2. Germs, meh. Fabric softener is icky though, so I try to wash that out.
