We have just had a monthly safety brief and part of it described how a fatality occured when an maintenance engineer used a EVOSTICK Foam Filler tube. (the ones you put into an tube gun)

As he was shaking it as stated on the instructions, the end cap blew off striking the ceiling and wall causing scoring to the surfaces. Unfortunately the tube also ( Read more... )
