The World :The world that Giotto and Tsuna come from is most easily explained by being called a crossover between the manga Shounen Dolls, and Hitman Reborn. Instead of hit men and the mafia what we have here is a secret orginization of special doll makers called Doll Masters.

What makes Doll Master different from regular doll makers you ask? Well Doll Masters who have the ability to use something called the Flame of Life ( Based off the Dying WIll Flame from Hitman Reborn ) to bring the dolls they create to life. The Flame of Life comes directly form the Doll Master's own life force, and because of that very few people can actually use it. When brought to life the doll will usually take on a personality that reflects their master's full potential.

The doll brought to life will remain like that until they are cut off from their master's flame. This usually means that the Doll Master will simply cut off the flame energy he gave their doll, however this can also happen if the Doll Master is in a life threatening condition, or has died.  There are actually many different types of the Flame of Life, each names after an "element" of the sky, or earth. When as doll master makes a doll that doll is imbued with that maker's flame type, and from then on can only be brought to life by a master with the same flame.

Story Summary:
In this world Tsuna's family isn't part of the mafia, but rather from a long line of doll makers. They own a shop with Tsuna is supposed to take over when he's an adult, though Tsuna him self isn't too keen on doing such a thing because owning a doll shop is not very manly... Not that many people consider Tsuna very manly in the first place. Reborn arrives, and turns his world upside down when he reveals to Tsuna that he has the "flame of life". A flame that can turn dolls into humans for an extended amount of time. On top of that Tsuna has the rare sky flame that can bring any doll to life. Giving Tsuna no choice in the matter he says he will be training the boy to become a Doll Master, just like his legendary ancestor.

Notes:
This AU actually runs a pretty close parallel to the KHR cannon. Things like how he meet his friends, and who his friends are, really very similar.
Please note that this AU wold is still under development right now. I'm still working out some of the kinks in it.

Doll Masters and The Flame of Life :

Doll Masters are quite like a secret society, or an underground group, because they're not known by the general public any more like they were about 400 years ago. In fact you won't even find them mentioned in history books, or anything of the sort. They've become things of bed times stories for children.

They also seem to be much fewer in number. It used to be that any person who made dolls was a Doll Master as well, this is not so any more in fact most people who make dolls today don't have the power to be a Doll Master. Only those with the power known as The Flame Of Life, and the ability to turn dolls to a human form can be called a Doll Master. Most doll masters have one doll called their companion that they keep with them always, and like to bring to life the most. These dolls are not only used as companions, but protectors as well. The Flame of Life is quite rare to have these days. It's these flames that allow Doll Masters to give their dolls life, personality, and human form for a limited amount of time.

A Doll Master's flame is linked directly to their own life force. When a Doll Master dies their doll, if in human form, will turn back to a doll until a new master gives it life. There are actually many different types of the Flame of Life, each named after an "element" of the sky, or earth. When as doll master makes a doll that doll is imbued with that maker's flame type, and from then on can only be brought to life by a master with the same flame. Some dolls are brought to life more often the others by their Doll Master. These dolls are called Companion Dolls, and over time they become able to use the Flame of Life used to bring them to life to fight. However if the doll uses too much of the flame it is possible for it to turn back to a doll, or in the worst case scenario kill their Doll Master.

Legendary Doll Masters :
About 10 Generations back from Tsuna's time period Doll Master were now as few as they are now, and there were some that were even semi-well known. However one group of Doll Masters seemed to rise above the rest, only to mysteriously disappear after about five years. They were known as the Vongola. A group of seven men and their dolls who are said to have gone on grand adventures before they disappeared.

What is was that they truly did has been mostly lost over time as they became characters of children's bed time stories. Fact blurred with fiction until all but a very, very chosen few knew what had actually happened. When they all disappeared some said that they died, some say that they simply went into hiding from some threat that was too strong for them to defeat, and others say they knew they were no longer needed to be protectors, but would some day return when they were needed again. Despite the varying reasons as to why the Vongola Doll Masters disappeared one thing remains consistent. It's said that before they disappeared each of the Doll Masters made one final doll in their image, and left it to their families to take care of until there came a person that could bring them to life.
Another group of Legendary Doll Masters are the Arcoblaeno. They're known as the strongest Doll Masters in the world, but other then that very little is known about where they come from, or what their purpose is. Each member of the Arcoblaeno group can be identified by the pendant that they wear, a pacifier in the color of their flame.

Doll Giotto :

Giotto is the final creation of the legendary leader of the Vongola, Ieyasu Giotto Sawada. He was left in the care of Ieyasu's family, being passed down form generation to generation to the first born child. Until he reached Tsuna no one in the Sawada family, even those who could light the sky flame that ran in their family, was able to bring him to life. Since he was made to look exactly like his creator it is quite possible that he actually holds Ieyasu's spirit, or flame. However since he hand't shown any obvious signs of either of those things no one can be sure if he is or not. Actually since all of the last creations of the Vongola members were made to look like them it's possible that all those dolls have the spirit or flame of their creators in them. However it has yet to be proven, though there's a suspicious lack of investigation into the matter, where it would normally be investigated immediately and thoroughly as transferring your soul into a doll is considered a taboo.
