I feel your pain. Sleep is a dodgy, elusive beast, then it jumps out from behind a tree and face-hugs you unconscious at the worst times. It's hard to tame.

Does Dr. A think anything of the sleep problems, or does she think they're just physical?

Back issues + sleep problems are often related. It might be keeping you from getting into a deep sleep portion of your cycle. As you get back into being able to exercise that should help with energy levels and probably with getting into a good rested state.

Not that that's much help now. I do advise doing something mentally restful before getting to sleep. Light reading, music, meditation might get your mind ready to sleep better, unwinding your brain as best you can often helps.

How much iron do you get in a day? I know it sounds weird but I can tell when my iron levels get low; I get more exhausted and worn down than tired. Exhausted needs rest; tired needs sleep. Subtle but critical difference.

Improved health, eating, and exercise definitely has a good effect. Yay.

Sadly, I don't have much good advice in this department. Sounds like you're on the right track though. It'll be good. :)

Can I tell you how much I sympathize, especially with #3?

If the sleeping problems persist, I have a few books my sleep doctor had me read I can recommend that were very helpful to me. I understand when you say you know the difference between anxiety related sleeplessness and other types of sleeplessness. The hard part is finding out what's causing it.
