The sad thing is, I know exactly what transpired in the shop that had to lay this out. It was a nearly completed, perfectly serviceable, if not artistically challenging, two-column two-page spread before the manager came in, took a look over some poor hack's shoulder as he was making one last adjustment to the kerning, and "made a few suggestions to improve it". At that point, the designer's spirit died with a whimper, leaving naught but a hollow shell of despair and a hand to click the Print To PDF button, and there was no one left to care if the column breaks got munged.
No, I didn't see that happen at my work... today... around 3:45. ;)
