My personal theory is that Jack was just pulling the Doctor's leg, maybe including Jack actually having the nickname because the people that knew him as a young man thought it a funny reference.
But Jack changing himself to cope with forgetting, and then forgetting that he changed, and then changing back and being so full of wonder and joy about it -- that captures something that I always liked best about Jack, and which this whole "Face of Boe" thing undermines.
Being a head in a jar must be bad enough for an introverted person who dislikes direct contact with the world and prefers to exist in the realm or pure thought. For Jack... let's say, this "joke" left me completely horrified until I had my denial firmly in place.
