I may have some useful information as to what may have caused it... though I can not be certain. There are certain alchemic potions and devices that can give the illusion that one has changed gender, altering the perception of those around them, and even to the one afflicted if the mixture is potent enough. I'm not sure what exactly would be involved in the making of such a thing, nor do I know if the components exist within your world to make it. The only ting is it would have to be ingested in order for it to take effect... Is it possible Hizumi ate something that could possibly have done this to him?

Your idea has merit. However, to find something which would afflict Little Narumi and Hizumi in equal measure would be improbable. They've entirely different tastes...nearly exclusive, and all but without commonality.

My sister would probably know more of what kind of mixture could do such a thing, but in most cases unless the use is deliberate, the substance is usually put into food or drink, much like poison would be. They wouldn't have to have eaten the same thing so long as both contained the alchemic concoction

Yet... Were it bothersome to Hizumi to be in his situation, he would not be in the situation. He would not allow it. He is such a person.
Whatever his reason, it somehow bends to his advantage to be female, whether his hand is behidn it or not. The effect lingers because he wills it to. Fate itself bends to his whim, and renders him indestructible.
There is truly, in that regard, little reason to pity him.
