I was told two very interesting senarios that can cause it are bird droppings and being exposed in any way to the chicken pox.
A humidifier was one of the things that helped me and hot liquids helped with the caughing things up issue. I also did oxgen therapy instead of taking medications. I don't like having to many meds or chemicals pumped in me so this was an alternative I was given. They do it at some spas. It pumps pure Oxygen into your lungs. Not only did it help with the pneumonia but also the fatigue issue that comes with it.
I don't know if your Dr. would directly have a machine to do so since it's just making it's popularity rounds in the US but he may be able to point you to a respectable place.... other things along the same lines are

I know there is a place in san fran that has an amazing oxygen therapy program...
