i'd say McCoy...if it's Academy or later and centers around Jim, I would also be okay with Bones. If it's with Jocelyn or family or Georgia, I think that's when Leonard works best. I hope other people chime in, I'd love to know what others think!
With characters in general, I try to vary it, with the name changing mostly based on the formality of the situation and the people the character's with, or with the nature of the character's thoughts--if they're thinking about personal and/or romantic stuff, I'm more apt to use a first name.
I may be in the minority, but...I really hate the name "Leonard". It sounds like something his mom/grandma would shout at him when she's angry and trying to get his attention so she can punish him good.
That being said, I usually prefer when a writer uses "McCoy" or a nickname such as "Len" when it's from Bones' POV. I don't ever see Bones calling himself Bones.
And when I'm writing, I always have Bones refer to himself as "Leo" - I loved when ceres_libera used that nickname for him in Switch, and I carry it on in tribute to that fic XD
See, with the exception of "Switch" I can't stand the name Leo. It's not how "Leonard" is pronounced, so why in the world would he be called that? But I can totally see how you could hate the name Leonard as well - it iS very outdated, even at the name that McCoy was first created. I think I'm one of the weird people who just loves the hell out of the name xD

I can definitely see your point, and I had the same problem when I started Switch. What helped me is actually the Spanish pronunciation of "Leo" -- which is "Lay-Oh" so it DOES sound like Leonard. And that's how I say it in my head, too, most of the time.
