Mpreg is so not my thing but I've been craving fic where Castiel has swollen, tender, oversensitized nipples and sometimes mpreg does that so I gave this one a shot. Then there was lactation (I missed the tag!) and had to flee.

Show started when he was 26, and we've time-jumped two years ahead of now, so he's around mid-thirties. It's probably reasonable to assume that he was having sex at 16 or 17, so sure, it's possible.
