It start slowly - he's getting a little softer around the edges, putting on some weight and being self-conscious about it, but soon enough other symptoms start to show that can't be ignored, mainly, his growing udder.
He's trying to hide it at first, but when he's losing more and more of himself and his humanity (going on full vegi diet, gaining more weight, having trouble to speak clearly, having more cow traits - take it as far as you like), Sam figures it out and works frantically to cure him.
Dean's clearly in pain but he wouldn't (or can't) tell Sam what's wrong. He's been secretly milking himself to ease the pain on his tits but when his fingers won't function anymore, Sam figures it out and comes to the rescue.
It's up to author if it's established relationship or first time.
