Out of curiosity, what does your "I have massive medical phobia speech" sound like? I can imagine someone might think you were being hyperbolic about your fear and saying, "well no one *likes* IVs, she'll be fine", so I'm wondering if there's any better way of conveying, "You really want to take me seriously about this, because I will seriously flip out and be impossible to deal with."

Hope the hand heals as nicely as the knee seems to be! And hope the hospital staff learns from experience for the next knee.

It goes something like "I have a severe and life-altering medical phobia. At the moment I require a major dose of valium to be able to handle a blood draw I have been working hard on overcoming it for several years now but I keep having setbacks due to unpleasant experiences. I first have a panic attack that send my blood pressure up very high, then I faint due to vasovagal disorder that causes my blood pressure to plummet very fast. This drop is potentially fatal, so I need to you to work with me."

That sounds immensely clear to me. Sorry you had such a stressful time. Glad your knee feels better.
