Sam rushes Dean to the hospital after finding him doubled over in pain on the bathroom floor. The doctors, however, can't find anything wrong with Dean and think he is scamming for drugs. Sam and Dean suspect a hex or evil eye. Sam busts Dean out of the hospital and takes him to Ellen's house. Bobby arrives and finds that Ellen has doped Dean to control the pain but they still don't know what's wrong with him. Bobby says he's going to check out the last case Sam and Dean were on and see if he can find any connection to what's wrong with Dean. He tells Sam to go through everything they own and look for hex bags, totems, etc. Bobby returns having found nothing. Ellen tells him that Dean is not better, and that they have tried counterspells and even an exorcism.
Ruby shows up at Ellen's house. She can't get in because the house is consecrated. Ruby tells Sam that Lilith is doing this to Dean, trying to claim him early, and that she holds Dean's contract. Ellen draws a summoning circle on the back door that lets Ruby into the house to see Dean. Ruby tries to break whatever has a hold on Dean, but it's too strong. She tells Sam that he needs to come with her. Sam wants Ruby to take him to see Lilith, but Ruby says he's not ready. She adds that Dean is a dead man. Sam refuses to believe and and she leaves angry.
Sam tells Ellen that he is leaving alone to find Lilith. Ellen slaps him across the face and asks him when this is all going to end. Sam leaves anyway. Dean wakes to find Ellen crying beside his bed. She tells him that Sam has left. Dean already knew it. Ellen leaves Dean so they can both rest.
Sam is barreling down a country road in the Impala when a car overtakes him and blocks the road. It's Bobby. He says he's going with Sam to find Lilith. Bobby takes Sam to see a hermit demonologist named
Grace Van Holt. Grace doesn't believe that Lility bought Dean's soul. She says that Lilith traffics in kings, presidents, CEOs, not the rank and file. Sam tells her that is mother was killed by a demon named Azazel and then fills her in on the rest of the story. Grace walks away from him into another room and pours herslelf a drink. Bobby follows and she demands to know what Bobby has brought into her house. Then she tells Sam to go back to Dean and be with him while he dies - there's nothing else to be done. Sam pulls out a gun and demands that Grace tell him how to find Lilith. Grace bitterly tells him to find an unclen place and Lilith will come looking for him. Sam leaves, without Bobby.
Dean is lying in bed alone, eyes closed. Someone pulls up a chair and he says he wants to be alone. When he cracks his eyes he finds his father sitting there. Dean thinks he is hallucinating, but John takes his hand and begins talking to him. John wants to know where Sam is, and Dean confesses that he's lost Sam. John tells Dean that he needs to trust him.
Sam has found a mass murder siate and begins a summoning ritual. Ruby appears, but Sam is angry - he wasn't summoning her. He throws her against a wall just as a man in a business suit appears. He identifies himself as
Lilith's Agent and confirms that Lilith holds Dean's contract. He also says that Lilith respects the terms of Dean's contract and isn't doing anything to him. Instead, the agent accuses Ruby of causing Dean's illness. He says that Ruby believes seeing Dean suffer would turn Sam into what Ruby wanted, and also that Dean is getting in Ruby's way. Ruby denies it, but the agenst says that she got into the house, didn't she? That's old school witchcraft, Ruby's first calling. He also says that one of Ruby's cohorts is with Dean right now.
Ellen hears voices upstairs but when she goes into Dean's room, he is alone. She leaves him. Alone with John, Dean is getting worse. John says that Dean has to save Sam from making a deal for him.
Ruby says that she can't kill Dean, it's not her life to take. The agent says Ruby doesn't need to kill Dean, just tip him over the edge. Sam, panicked, turns to leave and get back to Dean but the agent says he'll never get there in time. Instead, he can help Dean by exorcising Ruby and sending her back to Hell. Sam begins the exorcism.
At Ellen's house, John is convincing Dean to kill himself and end all the bargaining. The flame in Ellen's fireplace glows blue and she realizes something is in the house, but she is knocked back when she tries to go up the stairs. Sam finishes the exorcism. Ruby's body is empty, dead. A shot rings out as Ellen reaches Dean's door. Inside, she finds Dean pointing a gun at a pile of black ashes on the chair besicde his bed. He has recovered.
The agent tells Sam that Dean's contract can't be revoked. Sam wants to know what Lilith wants with him - he's out of this war. The agent says there's only one thing Sam can do for Dean - when his time is up, Sam doesn't have to let him go alone.
Sam goes back to Ellen's to pick Dean up. He is recovered. They get in the Impala and leave.
