Mid-story author's notes. When the person feels they just HAVE to stop everything dead in its tracks to say something like, "OMG I have a dress like this too LOLLOLLOL!!11!!!" Or "Isn't he just the hottest thing ever? I'm drooling just writing this scene!" It's not cute, it's not funny, it's just plain irritating.
I agree with the made-up languages. That is so annoying! Just write it in italics or something and say they're speaking whatever-language-it-is. Odds are, the author is not J.R.R. Tolkien and cannot get away with it. D<
Also: I think there is no excuse whatsoever for including fangirl Japanese in a fic. A few scattered words, I'll tolerate; honorifics, I'll tolerate if they're reasonable. But when an author thinks s/he needs to write half the dialogue in Japanese, it drives me insane.
Akajsdf. Yes. I know exactly what you mean -- I think this shows up in RP a lot too (which is like ... idk, fanfic writing but with more than one author, I guess). It's something I have to look out for because I play Roxas. XD

Pet peeves:
Bad characterisation, bad spelling, bad grammar, the usuals.
One thing which really really bugs me is when people get characters' eye colour wrong. Just... no. Like characterisation, it's a key part of that particular character. It's especially annoying when they're attempting to write a big romantic scene; if they get the eye colour wrong my brain does a mental hitch and has to pause...
Also: "It's my first fic be nice" type things. That's just asking for trouble...
I think that there's only one situation in which it's okay to get the eye color a little bit wrong, and that's when you're writing fic for a really old anime -- one from back in the days when they hand-painted the cells, when black cell paint showed up as green. THEN it might be slightly more acceptable.
But basically the way I see it is -- why the hell do you need to refer to the character's eye color anyway? If we're reading fic in that fandom, presumably we know who they are.
AUs where the characters look nothing like their original counterparts.
Characters that change suddenly when they fall in love (in HP fic, Draco when he's paired with anyone, but most often Draco/Hermione).
Purple prose - too much description of eyes, hair color, etc.
When canon or characterization is abandoned/changed in favor of the author's OTP. Learn to write within the bounds of canon - it makes for a much more interesting pairing.
"First time" sex that's OMG AMAZING instead of awkward and weird.
People that say 'Bad summary sorry lolz', and nothing else. If you're going to take the time to write a fic, then you should take the time to accurately describe it for anyone that would care to read it. I've seriously spent a half hour just trying to think up a summary, when the story was all done and the title was picked and everything.
