I remember in LJ's genesis, we (users) didn't have anything like that on our radar...and I don't think LJ (proto-) staff and developers had really thought about it either. For YEARS.
The first time I remember any mention of categorizing LJ material as "18+ content" was sometime around 2014 or 15 (maybe?), well after a number of my friends had left LJ and the site's core/public functions and purpose had generally become irrelevant to me. I definitely wasn't thinking much farther than the end of my own nose, but you're definitely right: predators are *desperate* to get into any and every possible venue that gives them even the slightest opportunity to get access to children.
Your thoughts came strongly to light, for me, just a few days ago when the wife started doing the American Heritage Girls introductory course for "abuse detection, response, and reporting," which was a pretty extensive course that took her nearly all evening to complete. If only all groups and organizations could be so diligent.

When I started doing massage, I got lots of annoying calls from men. "You could advertise 'For Women Only'." That turned out to be the current code for lesbians. Oh well, over time the annoying men got tired of being hung up on...
