Isn't weird how sometimes doing something you don't really like still makes you feel good at the end of the day? Take yesterday, for example: I spent a couple of hours doing hardcore cleaning of the Mister's old room at my in-laws' house. It sucked. There is no reason something can't escape from an accretion disk, that is nothing but the detritus that accumulates around something in a strong gravitational field. Whatever is in the disk is actually in orbit around the central body. I'm guessing you are talking about a black hole here. The only reason things fall into it eventually is that they lose energy due to collisions with each other which lowers their orbit to where the disk is denser so the loose even more energy. The event horizon is the point of no return.

The tidal stretching of things only occurs near the event horizon and if the black hole is large enough not even there. The only reason we "see" black holes is that the accretion disk gets hot due to the friction from the collisions. The gravitation energy is converted into heat. Quasars are accretion disks.
