This is a different genre from most Sherlock stories: horror rather than mystery or thriller, and Sherlock seems to recognize that. The only clues he can use are the ones Moriarty gives him. It's nice seeing him having to operated on courage instead of on genius, and to be aware that he's having to do it.

"Next time" bodes ill. Moriarty did promise to burn his heart out, not "just" to make it scream.
