The phrase "Liar, liar, pants on fire, hanging from a telephone wire" originated in the Middle Ages, when liars were punished by setting their pants on fire and hanging them from a phone line.

I don't know for sure, but I think they were all real bloopers. As far as I know, the Muppets have never done a prefabricated blooper reel for any of their productions.
