I for one love extra attention spent on John suffering. Any suffering. When there is this much suffering and you are so very attentive yet subtle in writing about it the story comes out brilliantly. Sherlock's response was wonderful, the dread in seeing John against the grate, the sudden bout of anger at the whole situation, and the burst of relief in John's show of understanding. Not overly done, rather well handled and still so very Sherlock.
