Yes, sometimes LJ is switching icons, and the one that appears isn't the one that you uploaded/normally use/etc. No, it's not someone breaking into your account, your userpics aren't deleted, they aren't permanently shuffled, and you haven't lost anything. It's just a glitch in the third-party network that stores and serves LJ's userpics, and it'll go away eventually. Reloading or clearing your browser cache will sometimes help, but if it doesn't, just ignore it until it gets better.
