1) Firstly, can we just get an Amen for Jared? Because HOLY CRAP, SON. Kill me softly, why don't you. *sobs and sobs*
2) My mom used to wake me up in the morning with that 'Good morning, good MOOOOOORNING' song. *SHUDDER* I honestly believe that this is partially why I so violently hate mornings to this day. OH, SAM. *clings*
3) I love the first scene between Dean and the DR. Dean woke up and Sam was GONE, you guys get that, right?! The way he blew in there ready to just take everybody OUT, and then just totally deflated, like he couldn't deny anymore that his one last final worst fear was coming true, uugh, my heart.
4) Maggots are my insta-puke trigger. My ONLY insta-puke trigger. I have seen things that would make most people pass out and thought to myself 'When's lunch?', but maggots? *PUKEPUKEPUKE* So, anyway, I was glad I was purposefully slightly drunk before this episode, because my exact response was 'If I wasn't so chilled out right now, I would puke.' (Hee.)
5) Faith reverse/rewind. *SNIFFLES* (Also, 2/3 times they were in Rufus's cabin - Rufus was buried, and Rufus knew all of Bobby's research - maybe the helpful ghost thing is Rufus?)
6) Sam is my hero. I just can't. I'm all out of cans.
7) Unpopular opinion time? I LIKE that Cas has been happy and cared for and safe all of this time. (Why the hell is that an unpopular opinion? Oh right, because fandom is cray cray. And can we STFU about poor Jimmy, Jimmy has been dead and gone for AAAAAAGES, srsly.) But anyway the montage where he remembered who he was? CHILLS. GOOSEBUMPS. ORGASMS. WHATEVER. (Also, Clarence! \o/ ) AND DEAN HAS BEEN KEEPING THE TRENCHCOAT IN EVERY CAR, I DO NOT KNOW WHAT MY FEELINGS ARE DOING.
8) I knew that Cas wouldn't be able to just fix Sam. But O.O. O.O. *FLAIL* I can honestly say I did NOT see that coming. But I do believe it was the right thing to do. Cas taking whatever damage this will cause him (Although he said he'd be fine, and I'm not convinced he can't slowly heal himself, Angels are not People) willingly to fix what he broke is no different from Sam taking in Lucifer in the first place, and it baffles me that his 'fans' aren't proud that he was willing to make that sacrifice. (Especially for Sam, who was the one to reach out and believe in him even AFTER he broke the wall in the first place!) Now. All of that being said - CAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAS!!!! *dramatic sobbing*
9) HOLY EPIC SPECIAL EFFECTS SEQUENCE BATMAN *applauds* Something about the "Sam!?" "Dean!" "SAM?" killed me a little okay a lot omg why Show why you have to do that every week I love hate you so so much rn.
10) Kudos to wardrobe for that end scene - Jared managed to somehow make himself seem very small and frail and underweight for most of the episode, but letting out his jacket and over shirt a little was a nice touch. But OH, DEAN - I just - and with the - and how you - *helpless flail*
It hurts me so much how these guys can never catch a break without losing someone. Dean has been flat-out suicidal ever since they lost Cas. Even the betrayal and everything Cas did wasn't enough to convince Dean not to give him another chance, and then he died, and it's just been Hell ever since. And to see Dean get a second chance to give Cas a THIRD chance and then STILL lose Cas, IT JUST SUCKS OKAY. But don't fool yourselves into thinking this is over. Cas will be back, Meg will be dealt with, this whole Crowley/Meg rivalry is just gearing up and Cas is the power play in the middle. We'll circle back to him.
As for Sam, here is my theory. Think of Sam's brain as a lake with a dam in it. The dam was built to hold back Sam's memories of Hell, because those memories would drive him mad. Cas cracked the wall, and the memories started flooding through - flashbacks at first - but then they started making him go mad - hallucinations. The wall kept on eroding until there was nothing left to rebuild. The hallucinations got stronger and stronger until Sam couldn't fight them any more.
Now, if Cas shifted anything from Sam to himself, there are only 2 options. 1) He took the hallucinations, or the crazy. This seems rather obvious since he then started having them himself. 2) He took the root CAUSE of Sam's problem, which would be the memories. This seems less likely, since the Hallucinations seemed to latch directly on to Cas personally. (Which again, I am alright with. And as
blacklid reminded me, Cas doesn't need to eat or sleep so he should be much better equipped to deal with it than Sam was.)
My opinion is that Cas took the crazy, but Sam still remembers Hell. If that is the case, then Sam SHOULD start to slowly go crazy AGAIN unless Cas somehow managed to take their power away, but the way he said it wasn't something he could make go away, and this would just get Sam back on his feet, makes it seem probable that Sam will eventually relapse.

ETA #2: OH AND. I don't buy Meg's army of one crap. I'm putting my money on the fact that she sent those demons to the hospital just to get Cas all powered back up again. I think she was hoping Cas would want to go kill Crowley, but now she's just going to have to wait and keep an eye on him. Oh, Meg. WHY ARE YOU STILL ALIVE. Also STAY AWAY FROM SAM. *shudders*
