Don't know if it's applicable to your case, but my experience has taught me that given enough time and anger, you *can* get what you are after. It might take a call to corporate HQ, but there is a person who can -- and wants to -- help you. Just have to find them and convince them that they want to.

Granted, the amount of time I'd spent had cost me more than the money that was in the balance, but I am morally satisfied.
